GENESIS Total Healthcare works in collaboration with many physicians groups and ACOs to decrease patient hospital readmissions, reducing healthcare costs, and achieve quality outcomes for patients who find themselves in acute or chronic health crisis.
Our goals are to first maintain, and then improve the patient’s functional status by assessing, monitoring, and treating patient’s symptoms under the direction of their physician. We utilize face-to-face care, telehealth and 24 hour dispatch to provide needed services. Our staff of nurses, aides, therapists and social workers are experienced and equipped to assist patients when care is needed. By being available, instructional, and timely.
Our patient-centered care model is team based and driven by quality and safety. We believe that it is not only what is wrong with the patient, as much as it is what matters to the patient. We increase patient’s capacity to self-mange their condition by thorough teaching on disease process, medication training and pain management, as well as ways to avoid crisis when possible therefore reducing the need for acute or emergent care. We believe when shared decision making is utilized through effective communication, empathy, and a partnership between the patient and the healthcare professional, improved patient outcomes and satisfaction are realized.
