GENESIS Total Healthcare, LLC is a Medicare/Medicaid Certified, CHAP Accredited, for-profit, faith-based, independently owned, comprehensive homecare agency in Genesee County Michigan. Located in Grand Blanc, Michigan providing homecare services to Genesee, Oakland, Wayne, McComb, Lapeer, and Saginaw Counties and surrounding communities. Owned and managed by clinicians and business professionals, our team includes experienced RNs and LPNs, Certified Nursing Assistants, Pediatric and Behavioral Health RNs, Physical and Occupational Therapists and Assistants, Medical Social workers, Companions and Sitters.
We provide care and services 24/7, 365 days a year. We work with area physicians groups, ACOs, all area hospitals, nursing facilities, and AFCs. We provide transition care from hospital to home for acute & chronic illness, case management and care coordination under the direction of PCPs, specialists, and surgeons. We have 24 hour monitoring and telehealth with 24 hour dispatch to provide urgent care as needed.
We also provide management support for other homecare services. We contract with Medicare, Medicaid, and with most HMOs and private insurances. We also work individually with families to provide Private Duty services for short and long-term care.
